Partnership Program
Criteria |
The purpose of the Chatham-Kent Safe Communities
Partnership Program is to increase community
involvement in injury prevention programming.
Up to $1000.00 per project is available to
support programs and activities that promote
injury prevention programming. Funds will be
awarded based on the applicant’s ability to
satisfy the following terms and conditions:

A completed application
package including project summary and budget
forms must be submitted by
February 12, 2007.
Applicants will be notified of the status of
their applications by
February 28, 2007.

Projects must focus on
promoting injury prevention programs
containing an educational component.

Projects must take place
within the Municipality of Chatham-Kent and
be completed by December 31, 2007.

A final activity report
is due 1 month after completion of the
project. This report should include the
evaluation form, a financial report, media
clippings, statistics regarding
participation and copies of any materials
produced.

Changes to the original
proposal will promptly be discussed with and
approved by the Chatham-Kent Safe
Communities Committee.

Funds are to be used in
accordance with financial and in-kind
eligibility guidelines (Appendix A).

Advertising, publicity
and resources associated with the project
must give appropriate credit. Recommended
wording “This project/activity has received
financial support from the Chatham-Kent Safe
Communities Committee”. Use of Safe
Communities logo will be in accordance with
its guidelines.
When reviewing project applications the Review
Committee will consider projects which include
the following key factors.

Program has clear and
measurable goals and objectives, and the
budget is feasible.

Program builds upon
existing community programs, or creates new
resources that are important to
Chatham-Kent, ie. does not duplicate
existing resources/programs.

Has organizational
capacity to provide leadership and project
direction.

Program activities are
publicized, monitored and evaluated.
-
Community partnerships
are formed or enhanced.
-
In-kind contributions are
provided by project partners.
Please provide a brief overview summarizing your
proposed project (no more than 3 pages), keeping
it short and to the point. Clearly state what
you intend to do, for whom, why, how and when.
At the top of each page, please include the
project name and sponsor/lead group name.
Priority will
be granted to projects that:

Demonstrate co-operation
between two or more local community
partners.

Have capacity to be
sustained in future.

Have clear and measurable
goals and objectives, a plan for project
evaluation and a feasible budget.
All recipients of a Safe Communities partnership
grant will be encouraged to join the
Chatham-Kent Safe Communities Committee to
ensure sustainability of Safe Communities
activities. |
